Home & Community Supports are personalized, one-on-one services that help children and adults with disabilities build independence, develop daily living skills, and participate more fully in their homes and communities.

Services are provided in the individual’s home and community, based on their needs, interests, and personal goals.

Support is provided by trained BILT staff who work directly with the individual on daily living skills, community participation, communication, recreation, and other areas of independence.

Goals are developed collaboratively with the individual, their family, and the BILT team. They are based on the individual’s strengths, needs, interests, and the areas in which they would like to become more independent.

To get started, complete the BILT Client Intake Request Form. This information helps us understand your needs and determine the appropriate next steps. After reviewing your request, a member of the BILT team will contact you regarding required consents, supporting documents, availability, and the intake meeting.
